Job Summary:

The Housing Navigator for the Enhanced Care Management (ECM)-Community Supports (CS) program focuses on addressing the housing needs of our students to support their overall well-being. Experience in housing services, case management, and a passion for advocating for the needs of vulnerable populations. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations. This involves the coordination of medical, behavioral, and social services to address the full range of patient needs, focusing on those with complex and chronic conditions. ECM programs are designed to optimize care delivery, reduce unnecessary hospitalizations, and improve the overall quality of life for students. The Housing Navigator plays a vital role in the ECM program, focusing on addressing the housing needs of our students to support their overall well-being.


Essential Duties::
• Collaborate with the care management team to integrate housing solutions into the overall care plan for students.

• Act as the primary liaison between the ECM program and County Housing Services to facilitate housing searches and present viable options to students.

• Guide students through the housing selection process, providing support and advice to help them make informed decisions.

• Process and enroll students into the County Coordinated Entry System (CES), enter data into County’s CES electronic database ensuring they are effectively integrated into the housing support network.

• Maintain up-to-date knowledge of CES processes and requirements to streamline students’ experiences and access to services.

• Identify and arrange non-emergency/non-medical transportation options for students to attend housing-related appointments, viewings, and meetings.

• Accompany students to housing-related activities when appropriate to provide support and assistance.

• Identify, coordinate, and get approval for any necessary environmental modifications for accessibility.

• Document services within the agency’s Electronic Health Record within 72 hours of each service provided.

• Attend workshops, meetings, and trainings as requested by supervisor.

• Performs other related duties as assigned.


Education and Experience::
Required

• Bachelor’s degree in behavioral health, human services, public health, or related field strongly preferred.

• Two years’ experience in housing navigation, social services, or related field with a focus on housing support.

• Strong understanding of the housing sector, including affordable housing and fair-housing laws.

• Excellent coordination and networking skills to work effectively with a variety of community partners and service providers.

• Strong advocacy and interpersonal skills, with the ability to work compassionately with diverse populations.

• Problem solving skills and the ability to think critically to overcome housing barriers.

• This position requires local travel and the ability to navigate public transportation with students.

• Bi-lingual in English and Spanish is preferred.

• Required to use company vehicle as well as personal vehicle to visit county jails and transport clients to and from appointments and other locations as needed.

• Required to have car insurance, vehicle, and valid driver’s license

Certifications or Licenses:

• Required: NONE.

• Preferred: Community Health Worker certification through California’s HCAI Department.

• Drivers License

Skills/Abilities:

• Excellent interpersonal and communications skills and the ability to work effectively with a wide range of constituencies in the community and within the organization.

• Knowledge of contract parameters, objectives, milestones, and other deliverables.

• Knowledge of contracting process and associated local, state, federal and other regulations.

• Ability to integrate budgetary and service utilization data, forecast expenditures, and respond to ensure both effective provider utilization and budget compliance.

• Ability to multi-task, identify problems, provide recommendations to management teams, and implement any applicable systems.

• Ability to work constructively with diverse people and with parties that may have divergent perspectives and interests.

• Ability to clearly express concepts and direction, both orally and in writing, and to prepare complex reports, policies, and procedures.

• Understand and appropriately apply Amity policies and procedures and adhere to agency-wide practices and regulations.

• Strong oral and written communication skills.

• Proficient with software such as Microsoft Office Suite (Word, Excel. Email, Internet, etc.).

• Faculty must frequently lift and/or move up to 50 pounds.

• The noise level in the work environment is moderate but can be loud.

• The temperature in the work environment is usually moderate to cool but can be cold or hot.

Travel:

• May travel locally by vehicle.
