We are currently seeking a Full-Time Discharge planner-Case Manager with experience in psychiatric care to join our esteemed team. In this position you would obtain, record, and communicate important clinical information, and provide clinical services within a multidisciplinary team environment.
Freedom Behavioral Hospital of Monroe is a 42 bed acute care facility providing specialized care for adults with their mental health and behavioral health needs. Job Duties:
Assumes responsibility for management of the discharge plan for the patients stay throughout the program.
Contacts referral sources and family members in order to gather clinical information for the multidisciplinary team and reports to the treatment team.
Interacts with members of the medical/clinical team to provide a flow of communication and accurately documents in the medical record the findings and data that supports level and intensity of service rendered.
Functions as a member of the multidisciplinary team and assist in facilitating the treatment team process.
Works closely with outside agencies to place patients after treatment to manage continuity of care.
Facilitates communication to outside agencies to ensure that these patient rights, care, and needs are met.
PREFERRED KNOWLEDGE, SKILLS, AND/OR EXPERIENCE REQUIRED:
Education: Bachelor's Degree - preferably in a Social Services field
License: N/A
Experience: 1+ years case worker and/or discharge planning activities
