Director of Case Management
• North Richland Hills, Texas
• Employment Type: Full-Time
• Schedule: Weekdays, Days (No Weekends)
• Estimated Hours: 7–10 hour shifts
• Salary Range: $102,107 – $135,000 annually
• Estimated Monthly Salary: $8,509 – $11,250
• Sign-On Bonus: $10,000
• Incentive Bonus: Up to 17.5%
• Relocation Assistance: Available (case by case)
Overview
We are seeking a Director of Case Management to lead and optimize care coordination, discharge planning, and utilization management within an acute care hospital setting.
This is a high-impact leadership role responsible for aligning clinical and operational strategies to improve patient flow, resource utilization, and overall outcomes. The Director will partner closely with executive leadership and interdisciplinary teams to drive performance and ensure compliance with regulatory standards.
Responsibilities
• Lead all case management operations including care coordination, discharge planning, and utilization review
• Develop and execute strategies to improve patient throughput, LOS, and resource utilization
• Oversee staffing models, team structure, onboarding, and competency development
• Monitor department performance metrics and implement process improvements
• Ensure compliance with hospital policies, regulatory requirements, and documentation standards
• Collaborate with executive leadership, physicians, and ancillary departments to align care delivery goals
• Manage departmental budgets, productivity, and operational efficiency
• Foster strong relationships with internal and external stakeholders to support continuity of care
• Lead and develop a high-performing, collaborative case management team culture
Required
• Bachelor’s degree
• RN, LCSW, or LMSW licensure
• 3+ years of acute care hospital case management experience
• 2+ years of leadership experience in case management
Preferred
• Master’s degree in Nursing, Healthcare Administration, or Business
• Strong background in utilization management, discharge planning, and regulatory compliance
• Demonstrated success improving operational and clinical performance metrics
