Employment Type: Full-Time
Schedule: Days | Weekdays Only | No Weekends
Shift Structure: 7–10 hour weekday shifts (occasional on-call or extended days may be required)
Compensation:
• $117,423 – $155,000 annually (based on experience)
• Incentive Bonus: Up to 17.5% annually
• Sign-On Bonus: $10,000
• Relocation Assistance: Case-by-case
Overview
A large, fast-paced Level I Trauma acute care hospital in coastal South Carolina is seeking a seasoned Director of Case Management to lead hospital-wide care coordination, utilization management, and discharge planning operations. This role oversees a high-performing case management team supporting complex, high-acuity patient populations and reports to regional case management leadership.
This opportunity is ideal for an experienced RN or Social Work leader with strong executive presence, proven change management capability, and experience in large, high-volume hospital environments.
Responsibilities
• Provide strategic and operational leadership for all hospital case management functions
• Direct daily department operations, including staffing models, skill mix, FTE management, onboarding, and competencies
• Ensure high-quality care coordination and discharge planning with timely, accurate documentation
• Oversee utilization management and core case management practices
• Partner with finance and regional leadership to achieve operational, quality, and throughput goals
• Analyze performance data and lead sustainable performance improvement initiatives
• Ensure compliance with organizational policies, regulatory requirements, and standards of practice
• Establish strong working relationships with executive leaders, medical staff, nursing leadership, legal, and compliance partners
• Support interdisciplinary collaboration across the continuum of care
Required:
• Bachelor’s degree
• Active licensure as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW)
• Minimum 3 years of recent acute care hospital case management experience
• Minimum 2 years of case management leadership experience
Preferred:
• Master’s degree in Nursing, Health Administration, or Business Administration
• Director-level experience in a large or high-volume hospital setting
• Strong background in utilization management, care coordination, and performance improvement
Candidates with strong leadership backgrounds and large-hospital experience, including licensed clinical social workers, will be considered.
