Position Summary
The Direct Care Professional (DCP) provides direct supervision, personal care assistance, medication administration, and daily operational support to residents at Holbrock Estates at 3021 Garrison Blvd, LLC, an Assisted Living Facility.
This position is staffed through Mid-Atlantic Staffing under an agency contract. The DCP ensures resident safety, dignity, regulatory compliance, accurate documentation, and continuity of care, including during emergencies and
resident relocation.
Minimum Qualifications & Compliance Requirements
Health & Background Clearance
Free from tuberculosis, measles, mumps, rubella, and varicella (B2)
Physician/MD statement (B2)
Criminal background check completed within 30 days prior to employment (B3)
Required Certifications (Must Be Current)
CPR (by certified instructor)
Basic First Aid (by certified instructor)
Certified Medication Technician (CMT) – if administering medications
Assisted Living Facility Training
Naloxone Administration Training
Medication Technicians must be 18 years or older and renew certification every two (2) years
Required Training (Initial & Ongoing)
Safety & Emergency Training
Fire & Life Safety (including use of fire extinguishers) – Initial & Annually
Emergency Disaster Plans – Initial & Annually
Infection & Food Safety
Infection Control (standard & contact precautions and hand hygiene) – Initial & Annually
Basic Food Safety – Initial & Annually
Resident Care & Regulatory Training
Cognitive Impairment (Alzheimer’s/Dementia)
Initial:
5 hours (personal care staff)
2 hours (non-personal care staff)
Annual:
2 hours (personal care staff)
1 hour (non-personal care staff)

Competency Validation
Personal care service competence demonstrated to the delegating nurse within 7 working days
(CNAs and GNAs exempt)
Care Documentation & Electronic Health Records
iCanotes
All care documentation must be completed in iCanotes in accordance with Assisted Living Facility policies, state regulations, and agency requirements.
Required iCanotes Documentation Includes:
Daily progress notes
Resident move-in / admission
Resident move-out / discharge
Changes in resident health status or condition
Resident transfer to the hospital or emergency department
Resident return from the hospital or emergency department
Redline App
Incident reports must be completed in the Redline app accurately and promptly
Meal temperature logs must be recorded in the Redline app
Essential Duties & Responsibilities
Resident Care & Supervision
Provide direct supervision of residents
Maintain a safe, structured, and drug-free environment
Conduct routine room checks
Enforce visitation policies
Ensure residents comply with house rules and safety expectations
Meal Service & Nutrition Support
Receive meals from the contracted meal delivery service (The Charmed Table)
Log meal temperatures in the Redline app upon delivery
Serve meals to residents appropriately
If meals are not delivered at scheduled meal times:
Immediately notify the Assisted Living Manager (ALM)
Contact The Charmed Table to inquire about delivery status
Resident Appointments & Scheduling
Print daily iCanotes schedules for each resident
Provide residents with their schedules and review them daily
Work with residents to ensure participation in all scheduled appointments
Support attendance at medical, behavioral health, and required services
Medication Management
Receive, log, secure, and administer medications and delegated treatments in compliance with Assisted Living regulations
Maintain accurate medication documentation
Documentation & Reporting
Complete all required documentation in iCanotes and Redline
Report incidents, health changes, and unusual events promptly
Take phone messages and direct calls appropriately
Receive and direct visitors as required
Maintenance & Facility Concerns
Identify and report maintenance or safety concerns
Submit maintenance issues to White Glove Property Management per facility protocol
Compliance & Cooperation
Understand and abide by all facility policies and procedures
Cooperate fully with government officials, inspectors, and regulators during visits or inquiries
Notify the ALM or Facility Director of agency visitors or compliance-related matters
Emergency & Safety Responsibilities
Take appropriate action during emergencies
Participate in fire drills, disaster drills, and inspections
Remain with residents during emergency relocation until properly relieved
Physical & Work Requirements
Ability to remain alert and responsive during assigned shifts
Ability to assist residents during emergencies
Ability to remain on site with residents during emergency relocation

Rose Street LLC. Mid-Atlantic Staffing is the employer. Holbrock Estates at 809 N Rose Street
LLC has contracted with Mid-Atlantic Staffing to provide staffing services. All work is subject to
Assisted Living Facility regulations, state requirements, and agency policies.
