What You’ll Do
Translate scientific language into clear, understandable information to help patients take charge of their health
Assess patient needs and readiness to adopt healthier behaviors
Develop, implement, and improve educational materials and tools that inspire positive change
Conduct health education sessions with patients and families, and participate in community outreach
Assist in training and mentoring Community Health Workers
Collaborate closely with care teams to support patient health goals
Lead tobacco cessation programs and travel between service locations as needed
Qualifications
Bachelor’s degree in public health, Health Education, Nursing, or related field
Minimum 2 years’ experience in community health education, outreach, or similar roles
Valid driver’s license and current CPR certification
Excellent interpersonal and communication skills, with leadership and independent learning abilities
