Position Summary:
The Community Engagement Coordinator provides direct psychosocial support to our Residents and their families, including individual counseling, admission support, mental health transitions, and group support services. The Social Service Worker collaborates with Team Members and community agencies to support Resident well-being and adjustment to our Long Term Care Home. Reporting to the Administrator, the position requires a registered SSW diploma and at least two years of relevant experience, ideally in a healthcare or LTC setting.
Responsibilities:
• Liaises with Social Service Workers in the community for the purpose of transitioning a resident back to the community. Liaises with community Mental Health Care services, offering resources and assisting with the transition of residents who have a mental health diagnosis
• Assists and supports new resident/substitute decision maker with admission into the Home. Provides one-to-one counseling where necessary and may refer to external resources for further counseling for the purpose of facilitating the resident’s adjustment to the Home, supporting psychosocial well-being, and supporting quality of life dimensions
• Provides counsel and therapy to assist residents and or family members to develop skills to deal with and resolve their social and personal problems, and a planned intervention basis. May also develop Support Group services for residents and family
• Facilitates internal resident transfers in collaboration with the resident/substitute decision maker, facility staff and other residents
• Supports the End of Life Program to include the facilitation of the Celebration of Life.
• Provides monthly reports on key indicators to the Administrator
• Performs any other related duties and projects as assigns by the Administrator
• Facilitates Spiritual and emotional support for the Resident as required.
• Diploma in Social Service Worker or Degree in BSW or MSW from a recognized College or University program
• Must be a registered member of Ontario College of Social Workers and Social Service Workers (OCSWSSW)
• Have at least 2 years of relevant experience in social work or other related field, preferably in health care/long term care
• Possess sound knowledge of social work methods and techniques
