Job Description

Updated New York State Education Department/Office of the Professions Licensure Requirements: According to the New York State Education Department, Office of the Professions/State Board Office for Social Work and Mental Health Practitioners, as defined in Articles 153, 154 and 163 of the Education Law, effective June 24, 2022, an individual must be:

1) Licensed as a Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW) to practice social work as operated by the Department of Mental Hygiene agencies. For additional information, please review 7704 Requirements for a license under NYS Social Work: Laws, Rules & Regulations: Article 154 (nysed.gov); or

2) Authorized through a limited permit valid for a period of not more than twelve months who has met all requirements for licensure as a licensed master social worker or a licensed clinical social worker except those relating to the examination and provided that the individual is under the general supervision of a licensed master social worker or a licensed clinical social worker, as determined by the department. The Co Response Team (CRT) Model is a collaboration between the New York Police Department (NYPD) and the Department of Health and Mental Hygiene (DOHMH), with a mission of serving individuals presenting with behavioral health challenges (mental health and substance misuse), as well as an elevated risk of harm to themselves or others. Teams consist of one behavioral health professional and two uniformed NYPD officers who engage individuals together in the community. CRT is deployed pre-and-post crisis to provide clinical assessments to determine service connection needs.

Duties will include but not be limited to:

- Provide de-escalation & assessments, interventions pre and post crisis.
- Deploy and maintain a caseload, conducting field work.
- Gather information, collaborate with other City agencies, community providers in real time to determine appropriate services.
- Troubleshoot challenges that may arise. Escalate to management as necessary.
- Participate in training for onboarding of new staff and meetings.
- All staff will be cross trained in deployments and Co-Response Intake (i.e., data entry, database searches, phone calls to referral sources).
- Collaborate with NYPD partners on deployments, outcomes and follow ups.
- Build relationships with other City agencies, community providers and relevant collateral contacts to coordinate appropriate responses.
- Coordinate with community service providers to connect clients to long-term treatment and support services.
- Collaborate with FDNY-EMT and local hospitals around involuntary hospitalizations and safety issues.
- Enter progress notes in program database(s) in a timely and efficient manner.
- Document all contacts with and on behalf of clients. Coordinate with other collaterals, as needed.
- Case management, such as, not limited to, submitting referral to services, follow up to ensure connection and engagement.
- Engage service providers and other clients support as needed.
- Participate in all program-related trainings and meetings.
- Maintain communication with supervisors while in the field accordingly.
- Maintaining professional behavior and conduct in the work environment is required.
- Other responsibilities as assigned by leadership.

Preferred Skills:

- Bilingual: (Spanish preferred) – (Highly preferred skill: language proficiency)
- Knowledge and experience with best practices around violence risk identification, assessments, crisis intervention, engagement of hard-to-reach clients, motivational interviewing, trauma informed care.
- A minimum of 2 years, full-time, direct service experience, post MSW, with highly vulnerable populations, people with mental health, substance abuse issues, criminal legal involvement and families.
- Experience conducting field work, home visits.
- Significant provider experience in behavioral health treatment and support services.
- Must have ability to establish and maintain effective working relationships with internal and external partners.
- Strong understanding of intersection of NYC’s criminal justice and behavioral health systems, including experience with behavioral health treatment providers.
- Flexible, adaptable, ability to work in a fast paced, professional environment. Ability to work in a team.
- Flexibility to provide operational coverage as needed, including working other shifts.
- Proficient in computers, including Microsoft Office suite.

Graduation from an accredited graduate school of social work as evidenced by a master's degree or certificate and four years of full-time paid experience in public health, medical or psychiatric social work, at least two years of which must have been in a supervisory, administrative or consultative capacity.
Within 18 months of the date of appointment, all candidates will be required to obtain a certificate as Certified Social Worker (CSW) issued by the New York State Department of Education.
