Position Overview
The Clinical Team Leader works under the direction of the Clinical Program Director to provide assessments, therapy, and treatment planning for individuals and families, Provides support to other team members, assist in building the program and is a great community partner. The ideal candidate will have strong clinical skills, effective communication abilities, and a passion for supporting individuals on their recovery journey, community engagement.
Key Responsibilities
Understand and abide by TPC Wellness mission and vision goals.
Ability to grow a program.
Provide community engagement and attend community events to help promote TPC Wellness.
Conduct initial and follow-up assessments to determine treatment needs.
Develop individualized case plans in collaboration with individuals served.
Facilitate individual, group, and family therapy sessions (under supervision as applicable).
Document progress, behaviors, and interactions in clear, timely case notes.
Monitor and track individual progress throughout treatment.
Communicate relevant information with staff, probation officers, and other professionals.
Participate in weekly treatment team meetings.
Create aftercare plans for individuals upon discharge.
Liaise with insurance companies as requested.
Assist with yearly audits and compile required documentation.
Transport individuals on occasion to appointments or functions.
Represent TPC Wellness in court when necessary.
Contribute feedback to improve programming and services.
Attend required yearly training and maintain compliance with licensure.
Collaborate with other departments and assist where needed.
Qualifications
Required Licensure: LICDC or LPC(must remain current and valid).
Valid driver’s license required.
Education:
Masters degree in Social Work, or Counseling
Skills & Attributes
Strong organizational and time management skills.
Proficiency with technology and clinical documentation systems.
Excellent communication and collaboration abilities.
Ability to handle demanding or crisis situations with professionalism.
Leader in the community
Ability to interact with people.
