Client Services Coordinator- I/D

BAYADA Home Health Care has an immediate opening for a Client Service Coordinator in our Blue Bell, PA Habilitation Home Care office.

Responsibilities for a Client Services Coordinator:

• Coordinate day to day services and schedules for individuals with I/DD

• Focus on managing coordination of client services and emergent scheduling issues

• Manage your Client Services Coordinator caseload while proactively growing it

• Build lasting relationships with clients, referral sources, payors and community organizations

• Develop strong, communicative relationships with the team

• Maintain effective fiscal management of your caseload by monitoring metrics and client authorizations

• Client Services Coordinators will partner with Clinical Managers to provide supervision and support to Habilitation Technicians

• In collaboration with the Clinical Manager, Habilitation Manager, Direct Support Professionals (DSP) and other teams to act as an advocate for clients in the Habilitation program

• Collaborate with the client, family, Case Managers, DSPs and other teams to implement and evaluate client scheduling needs

• Maintain ongoing communication with Case Managers/Care Coordinators and other stakeholders

Qualifications:

• Bachelor degree is required

• At least 1 year of experience with the I/DD population (Preferred)

• Prior medical office or home care experience preferred

• Prior supervisory experience a plus

• Demonstrated record of successfully taking on increased responsibility (goal achievement)

• Ambition to grow and advance beyond current position

• Strong computer skills required (electronic medical record)

• Excellent communication and interpersonal skills
