MULTI-EMPLOYER ROLE

This W-2 hourly Certified Peer Recovery Specialist position includes responsibilities that span across the following three affiliated organizations, all under shared leadership:

          FOCUS #1 Lighthouse Behavioral Health Center, LLC

          FOCUS #2 Community Wellness Psychiatric Rehabilitation Program

          FOCUS #3 Lighthouse Behavioral Health Center at 1114 E 20th Street, LLC

          FOCUS #4 Lighthouse Behavioral Health Center at 1116 E 20th Street, LLC

You will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all two companies, with hours, responsibilities, and compliance obligations tracked separately for each.

This role is structured as an up to 40-hour per week position, with approximately 10 of the total weekly hours allocated to Lighthouse Behavioral Health Center, LLC, Community Wellness Psychiatric Rehabilitation Program,  Lighthouse Behavioral Health Center at 1114 E 20th Street, LLC and  Lighthouse Behavioral Health Center at 1116 E 20th Street, LLC. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care, each employer will manage its own onboarding, HR, and compliance processes in accordance with applicable laws and organizational policies.

Client Service Location Expectations

This position is based on-site at the supportive housing program located at 1114 and 1116 E 20th Street, Baltimore, MD 21218. All services are to be delivered in person at this location. The staff member is expected to be present on-site during scheduled shifts, provide support within the residence, and ensure documentation and responsibilities are completed in alignment with program protocols. Remote work or off-site service delivery is not applicable for this role.

Caseload Makeup

The assigned caseload for this position consists of eighteen (16) Category A clients who reside in the community and may be serviced at 3310 Eastern Avenue, Baltimore, MD, throughout the community, or in the community meeting room of 1114 and 1116 E 20th Street location. 

DIVISION: Operations

CLASSIFICATION

This is a full-time, W-2 employee position totaling 40 hours per week, with hours allocated across two affiliated organizations as follows:

Community Wellness Psychiatric Rehabilitation Program (CWPRP):

Part-time, W-2 employee — approximately 10 hours per week (25% of total weekly hours)

Lighthouse Behavioral Health Center (LHBHC):

Part-time, W-2 employee — approximately 10 hours per week (25% of total weekly hours)

Lighthouse Behavioral Health Center at 1114 E 20th Street, LLC :

Part-time, W-2 employee — approximately 10 hours per week (25% of total weekly hours)

Lighthouse Behavioral Health Center 1116 E 20th Street, LLC :

Part-time, W-2 employee — approximately 10 hours per week (25% of total weekly hours)

While each assignment is classified as part-time under its respective entity, the combined total constitutes a full-time, 40-hour per week role across the organization network. This structure ensures shared staffing efficiency and cross-program collaboration in alignment with operational and regulatory needs.

WORK SCHEDULE: Every Monday, Tuesday, Wednesday, Thursday and Fridays of each week from 8:00 AM to 5:00 PM each day.
 
COMPENSATION RANGE
Compensation for this position ranges from $25.00 to $15.00 per hour, and is commensurate with experience, expertise, verified credentials, and available company budget.

ANTICIPATED TRAVEL: 5% of the time. There is no additional compensation for travel.
 
GENERAL DUTIES AND RESPONSIBILITIES

Assist supervisor with work-related requests.

Collaborate with and serve as program POC for external treatment providers and outside agencies as needed.

Responsible for following regulations of COMAR, CARF, any other regulatory body, and company policies/ procedures related to service delivery and documentation and co-facilitating orientation of all new hires to these as well.

Support the maintenance of a safe environment by participating in training and drills as requested.

Maintain confidentiality of records relating to client’s treatment.

Participate in external and internal audits/surveys (CARF/CSA/OHCQ) as directed by the supervisor.

Participates in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.

SUMMARY OF POSITION RESPONSIBILITIES

The Certified Peer Recovery Specialist provides recovery-based support, medication administration, and direct care to residents in a licensed residential treatment program. This role combines peer recovery, Certified Medication Technician (CMT), and House Manager responsibilities to support clients with co-occurring behavioral health and substance use disorders. Staff work under the supervision of Operations Manager 5 or Operations Supervisor 5, and collaborate with clinical, medical, and administrative teams.

SCHEDULED DUTIES AND RESPONSIBILITIES

Peer Recovery Support & Resident Engagement

Provide trauma-informed, person-centered peer support to residents based on shared recovery experiences

Facilitate or co-facilitate groups, recovery check-ins, and therapeutic recreational activities

Support residents in crisis de-escalation, relapse prevention strategies, and coping skills

Encourage treatment participation, goal setting, and progress in recovery plans

Medication Administration (CMT Duties)

Administer prescribed medications according to physician orders and delegating nurse supervision

Accurately document medication administration in Medication Administration Records (MARs)

Report refusals, side effects, discrepancies, or adverse reactions to clinical or nursing staff

Ensure proper storage, labeling, and counting of medications, including controlled substances

Facility Oversight & Household Duties

Monitor resident behavior, headcounts, and maintain visual supervision as assigned

Ensure common areas, resident rooms, and bathrooms remain clean and safe

Assist with meal service, kitchen tasks, trash removal, and light housekeeping duties

Monitor and report facility maintenance or safety concerns promptly

Treatment & Documentation Compliance

Support implementation of individualized treatment and service plans

Participate in monthly reviews of resident treatment progress and provide peer-based input

Assist with initial and ongoing assessments and diagnostic follow-ups every six months

Submit daily care notes in the electronic health record system to confirm that the client is occupying their assigned bed space. These notes must be coded as RESRB

Maintain logs, shift notes, and communication forms in alignment with documentation standards

Shift Leadership & Communication

Participate in a formal shift change at both the beginning and end of your shift with off going and oncoming staff

Shift change must include:

Medication counts and verification of the med cart log

A briefing on residents currently off-site (e.g., hospital, home pass, appointments)

Notification of any visitors currently in the facility

Communication of any incomplete tasks or follow-up items

Updates on resident behavior concerns, incidents, or PRN medication use

Confirmation of supply levels, safety issues, and facility concerns

Contact the Operations Manager 5 or Operations Supervisor 5 immediately in the event of any incident, the arrival of a state surveyor, or if emergency personnel are on the property

Communicate clearly with the clinical team, families (as appropriate), and within interdisciplinary meetings

Health, Safety & Emergency Response

Follow infection control, safety, and universal precaution protocols

Respond to behavioral and medical emergencies using approved procedures

Support residents with CPR, first aid, or transport to care when necessary

Participate in emergency drills and follow evacuation or lockdown procedures as directed

The Psychiatric Rehabilitation Program (PRP) is a core service of Community Wellness Psychiatric rehabilitation Program, designed to support individuals living with serious mental illness in achieving stability, independence, and improved quality of life. Our PRP provides structured, community-based support focused on developing life skills, managing symptoms, and promoting personal recovery goals.

Through individualized service planning, case management, health home coordination, and peer support, the PRP helps clients navigate housing, employment, education, and health care access. Services are person-centered, trauma-informed, and culturally responsive, with an emphasis on whole-person care and long-term wellness. The program serves both youth and adults, working in close collaboration with clinical providers, families, and community partners.

SUMMARY OF POSITION RESPONSIBILITIES
The Direct Support Professional (DSP) in the Psychiatric Rehabilitation Program (PRP) is responsible for supporting individuals with serious mental illness and/or co-occurring disorders in achieving greater independence and community integration. The DSP helps clients build daily living skills, access resources, follow through on treatment goals, and develop coping strategies that support their wellness and recovery. Services may be provided in the community, in the client’s home, or at a designated program site and must align with the client's individualized rehabilitation plan.
 
SCHEDULED DUTIES AND RESPONSIBILITIES

Provide skill-building and rehabilitation services in alignment with each client’s Individualized Rehabilitation Plan (IRP), as scheduled by the Scheduling Department and in response to client needs.

Assist clients in developing and practicing activities of daily living (ADLs), including cooking, cleaning, personal hygiene, transportation, budgeting, and other life skills that promote independence.

Support clients in navigating community systems, such as public transportation, benefits enrollment offices, housing resources, and medical or behavioral health appointments.

Deliver services in accordance with PRP scheduling protocols, client availability, and the goals outlined in the IRP.

Promote client independence, self-advocacy, and recovery-oriented practices through every interaction.

Submit accurate and client-centered daily progress notes in the electronic health record (EHR) that reflect the nature and outcome of each session.

Complete all session documentation in real time or no later than the end of each shift.

Follow all documentation standards and use the appropriate service codes for PRP sessions:

Use H2016 with Place of Service (POS) code 15 for off-site sessions (minimum of 15 minutes)

Use H2016 with POS code 52 for on-site sessions (minimum of 60 minutes)

Use modifier U2 for services provided to minor clients

Use modifier U3 for services provided to adult clients

Maintain minimum service delivery requirements:

At least 3 sessions per month for minor clients

At least 6 sessions per month for adult clients

Render services in accordance with the assigned schedule provided by the Scheduling Department and remain responsive to emerging client needs requiring additional or flexible support.

SUMMARY OF POSITION RESPONSIBILITIES

The House Manager at a Licensed Assisted Living Facility provides essential daily care, medication support, and supervision to residents in accordance with the Maryland Department of Health’s COMAR 10.07.14 regulations. SCHEDULED DUTIES AND RESPONSIBILITIES

Resident Personal Care & Support

Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, mobility, and hygiene

Provide meal assistance, hydration reminders, and monitor nutritional intake

Assist with transfers, use of assistive devices, and fall prevention practices

Engage residents in social, recreational, or wellness activities

Medication Administration (CMT Duties)

Administer medications in accordance with physician orders, the resident’s service plan, and under the supervision of the delegating nurse

Document medication administration on paper or electronic Medication Administration Records (MARs)

Observe and report medication refusals, adverse reactions, or errors immediately

Ensure medications are stored securely and counted per protocol (including controlled substances)

Facility & Household Duties

Ensure cleanliness and organization of resident rooms and common areas

Complete light housekeeping, laundry, trash removal, and restocking of supplies

Assist with meal service, dietary accommodations, and kitchen sanitization

Monitor for maintenance issues and report facility concerns to management

Bed Service Fee Protocol
Ensure that clients pay their bed service fees directly to the Accounts Receivables (A/R) Department, as outlined in their lease agreement. House managers are not permitted to collect fees directly from clients. Instead, clients should be encouraged to submit payment via one of the following approved methods:

At an authorized PayNearMe location using their designated payment slip

By delivering the payment to the A/R Department Manager at 3310 Eastern Avenue

By submitting payment to a designated Special Police Officer

By submitting payment directly to the Operations Manager

UNSCHEDULED DUTIES AND RESPONSIBILITIES

Respond to emergencies, facility issues, or behavioral escalations in real time

Support tenant admissions and discharges, including room readiness and orientation

Participate in audits, inspections, quality assurance reviews, and licensing site visits

Accompany tenants to appointments or community services when required

Cover additional shifts or provide emergency coverage as needed

Maintain confidentiality and uphold trauma-informed, culturally responsive service delivery practices
