Salary Range: $55,000- $60,000
Neighborhood Self Help by Older Persons Projects (SHOPP) is a dynamic and innovative non-profit organization that supports older adults, caregivers, and individuals with disabilities. SHOPP offers 12 essential programs, including CCNP (Community Care Network Program), 4 Older Adult Centers (Casa Boricua, Leon, Soundview, and Guess), NORC (Naturally Occurring Retirement Communities), Pop Up Café, Case Management with Friendly Visiting, Elder Justice, Caregiver Support Program, and NY Connects. Job Description:
The Neighborhood SHOPP Case Management Program aims to maintain and assist homebound older adults living independently in the community and reduce social isolation. The Case Worker will serve as a valuable resource to supplement staff resources and meet the organization's needs. Services but not limited to include identifying needs through a comprehensive intake and assessment process and determining eligibility for NYC Aging-funded in-home services such as home-delivered meals, EISEP, and other supportive services. The Case Worker will also manage a caseload when staff coverage is needed and will assist periodically with expanding community engagement efforts to establish connections for Friendly Visiting recruitment. The Case Worker reports directly to the CMP Assistant Program Director.
Requirements:
The position requires a Baccalaureate Degree in Social Work from an accredited school and experience in a social service setting. This position requires a strong understanding of the importance of volunteer programs and their impact on nonprofit organizations, supporting individuals who may be isolated and improving the quality of life for individuals. Essential competencies, knowledge, and skills include advocacy, care coordination, communication, problem-solving, collaborative teamwork, dedication to empowerment, and strength-based social work practice. The ability to work with a diverse population and showcase the ability to work independently and collaboratively, displaying problem-solving skills. Bilingual skills in Spanish is recommended. Computer literacy is mandatory, and knowledge of STARS is also helpful. The Case Worker position requires fieldwork in the South Bronx, which may require walking up and down stairs and working outdoors in different weather conditions. All staff must be able to travel and report to the office at least 2-3x a week or otherwise requested and attend in-person training, meetings, or events.
The CMP Case Worker is specifically responsible for the following:
• Establish helping relationships with participants of the agency;
• Elicit and evaluate the client’s presenting problem;
• Determine program eligibility and appropriateness;
• Prioritize intakes that present crisis and bring the information to the attention of the PD/AD;
• Document the details of the preliminary screening in STARS and submit the intake for
assignment;
• Complete accurate EISEP Home Care cost-share worksheet tool;
• Make necessary referrals via calls to vendors as well as through STARS;
• Conduct the 24 hr./ 15-day follow-up phone calls for the initial authorization of Home Delivered Meals;
• Monitor the Queue daily for missed meals to monitor the client’s safety;
• Comply with the NYC AGING Case Management Standards when interacting with consumers and performing other job functions;
• Maintain and complete well-written documentation in the client's case records;
• Complete worker logs/ daily units and other administrative requirements;
• Participate in SHOPP staff training programs, staff meetings, and other meetings as assigned;
• Communicate and collaborate with service providers;
• Provide information and referral to other SHOPP programs and appropriate community agencies for resources and assistance;
• Maintain a short-term caseload, perform initial assessments of the client's needs, and develop
care plan goals when coverage is needed;
• Assist older adults with Benefit and Entitlement Assistance;
• Collaborate with SHOPP Outreach team and Volunteer Service Manager to assist with
expanding volunteer efforts;
• Participate in outreach efforts, such as Community Fairs and Community Board meetings, etc.

• Involvement in a supervisory relationship with the PD/AD and Volunteer Service Manager
(VSM);
• Perform other tasks as assigned by the Leadership team.
