CREST provides rapid access to short-term, recovery-oriented crisis services to address both crisis intervention and stabilization for adults experiencing a mental health and/or substance abuse crisis. Our crisis services help people as they navigate their crisis, overcome obstacles, and plan their ongoing mental health needs

Position Title: Case Manager
Position Type: Full Time; 40 Hours Weekly

Responsibilities:

• Participate in a multi-disciplinary team
• Engage all consumers in treatment planning, WRAP planning and all areas of service delivery
• Assist consumers with activities of daily living
• Provide direct assistance in obtaining basic needs for consumers
• Provide on-going assessment of client's emotional well being, clinical symptomology, behavioral changes and medication compliance

Requirements:
• Bachelor's Degree in a Human Services field
• Experience with Crisis Intervention preferred
