Position Summary
The Case Manager II serves individuals and families by assessing needs, connecting neighbors to appropriate resources, and supporting progress toward self-sufficiency. This role combines direct neighbor services with leadership responsibility for program design, implementation, and outcomes.
The Case Manager II also leads the coordination and execution of key programs, ensures compliance with grant and organizational requirements, and maintains high standards for data integrity, reporting, and service delivery.
Neighbor Services & Case Management
Conduct neighbor intake, assessment, and eligibility determination to identify appropriate services and resources
Meet with neighbors regularly to assess needs and provide individualized support
Work with neighbors to develop and follow individualized plans with clear goals, action steps, and timelines
Provide ongoing coaching, accountability, and follow-up to support neighbor progress toward stability and self-sufficiency
Apply wraparound and two-generation approaches to address immediate needs and long-term outcomes
Respond effectively to urgent or changing neighbor situations with appropriate interventions and referrals
Maintain accurate, timely, and complete case documentation to support service delivery, reporting, and compliance
Collaborate with internal staff and external partners to coordinate services and avoid duplication
Participate in neighbor service team meetings and contribute to coordinated case planning
Maintain strong working knowledge of community resources and referral networks
Maintain appropriate professional boundaries while supporting neighbor self-determination and independence
Program Leadership & Coordination
Design, coordinate, and manage self-sufficiency programs, including employment readiness, financial literacy, life skills development, and health and wellness initiatives
Plan and execute seasonal and special programs, including Giving Tree initiatives, Back-to-School programs, and other community engagement activities
Build and maintain partnerships with employers, training providers, social service agencies, and community organizations
Ensure effective program implementation, including scheduling, facilitation, and participant engagement
Create and manage program processes, materials, and documentation
Develop, administer, and evaluate program outcomes, including participant feedback and program effectiveness metrics
Coordinate participant logistics, including attendance tracking, materials distribution, and incentives
Lead recruitment of program participants and coordinate outreach efforts
Ensure programs are delivered consistently, effectively, and in alignment with organizational goals
Data, Reporting & Compliance Management
Maintain the integrity and completeness of neighbor and program data
Leverage reporting to monitor program performance and outcomes
Ensure compliance with grant and organizational requirements, including budgets, timelines, and documentation
Track and report on program expenditures and outcomes
Deliver required internal and grant reporting
Community Engagement & Partnerships
Develop and sustain relationships with community partners and service providers
Coordinate referrals and collaborative services for neighbors
Represent the organization in outreach and community engagement efforts
Identify opportunities to expand partnerships and program reach
Secure and prepare neighbor participation in storytelling and outreach efforts
Volunteer Coordination
Recruit, train, and supervise volunteers supporting neighbor services events and programs
Coordinate volunteer roles to ensure effective program delivery
Provide ongoing guidance and support to maintain volunteer engagement
Organizational Support
Participate in internal committees and cross-functional initiatives
Provide leadership and backup support within the neighbor services team
Support special events and organizational activities, including evenings and weekends as needed
Qualifications
Required
Strong interpersonal and communication skills
Ability to lead programs and manage multiple priorities
Ability to work effectively with diverse populations
Strong organizational and problem-solving skills
Ability to respond to changing priorities and emerging needs
Proficiency with standard office software, data management, and reporting
Two to four years of experience in social services, case management, or a related field
Preferred
Degree in Social Work, Behavioral Sciences, or related field
Experience in social services or nonprofit program management
Experience designing or managing programs
Knowledge of community resources and systems
Bilingual (Spanish preferred)
Position Details, Compensation & Benefits
Status & Schedule: Full-time (40 hours/week), non-exempt, primarily onsite with limited hybrid flexibility. Includes regular evening hours and occasional weekends.
Work Environment: Tri-Lakes Cares values sustainable schedules and strives to balance dependable service coverage with flexibility in a family- and life-friendly environment.
Compensation: Target hourly rate of $26–$30, depending on experience.
Benefits & Time Off: SIMPLE IRA with 3% employer match; health insurance with 50% employer subsidy for full-time employees; optional dental and vision coverage (employee-paid). Vacation and sick leave accrue based on hours worked, along with 11 paid holidays annually.
