Job Description
Housing Navigation Case Manager Goal:
Engage with individuals and families experiencing homelessness to identify barriers to housing, prioritize a housing waitlist, and refer and link to permanent housing, contributing to the goal of helping them exit the homeless system.
The Housing Navigation Program Case Manager reports to the Housing Navigation Program Manager and is responsible for the following:
Case Management Responsibilities:
Case Management:
Manage the caseload of assigned consumers, ensuring their overall support.
Complete and maintain documentation in both consumer files and the New Jersey Homeless Management Information System (HMIS) promptly.
Build familiarity with each assigned consumer, maintaining contact and understanding their life roles and needs.
Meet consumers at the MHAMC offices or in the community to establish positive rapport.
Assist consumers in housing searches, completing applications, and developing stabilization plans.
Assess consumer needs and provide appropriate referrals and linkages to community services.
Team Collaboration and Meetings:
Attend peer supervision meetings and agency staff meetings regularly.
Participate in meetings with program partners and community providers to achieve program goals.
Skills Required:
Ability to assess clinical status and interpret consumer charts.
Capacity to identify and handle clinical crises requiring emergency intervention.
Strong engagement skills with consumers and the ability to build rapport.
Networking abilities with community resources and supports.
Knowledge of mental health and substance use issues to educate consumers.
Safe driving habits in accordance with Motor Vehicle report guidelines.
Awareness of clinical resources and how to access them.
Ability to create housing stabilization plans for consumers.
Clear communication skills, both written and verbal.
Education Requirements:
Bachelor’s degree in social work or a related human services field with 2 years of relevant experience. A lower degree with additional experience or lived experience may be considered in lieu of formal education.
This role focuses on direct engagement with consumers, supporting their housing search, and ensuring they have access to the resources they need to transition to stable housing.
