Case Manager/ Discharge Planner

Freedom Behavioral Hospital of Plainview, Texas is a 20 bed, acute geriatric psych facility located in Plainview, Texas. We are looking for an experienced Case Manager/Discharge Planner. Previous psychiatric experience in an acute setting is a must. Strong preference given to previous geriatric psych experience. Teamwork, team development and team training are all key qualities for this position. 

 The Case Manager/Discharge Planner, in accordance with the Joint Commission, federal, and state regulations, Freedoms' mission, policies and procedures and PI standards, is responsible for coordinating with the admission staff and clinical staff to facilitate the meeting of patient's treatment needs. This position assumes responsibility for management of the discharge plan and utilization review activities for the patients stay throughout the program. 

The position contacts referral sources and family members in order to gather clinical information for the multidisciplinary team and reports to the treatment team.  The Case Manager/Discharge Planner interacts with members of the medical/clinical team to provide a flow of communication. This position accurately documents in the medical record the findings and data that supports level and intensity of service rendered.

This position is a member of the multidisciplinary team and assists in facilitating the treatment team process. The Case Manager/Discharge Planner is the primary source for payer source contact and liaisons with the medical and clinical staff in order to communicate admission and continued stay criteria to referral sources, families, and payer sources as needed. Communicates with patients, families, and referral sources to ensure the positive treatment outcomes.  In addition, maintains performance improvement activities within the department and participates QM activities. The Case Manager/Discharge Planner adheres to the highest ethical standards regarding patient care and rights and facilitates communication to outside agencies to ensure that these patient rights, care, and needs are met.
