Service & Support Administrator
(aka: Case Manager)
Full-Time | Monday–Friday | 8:00 AM–4:00 PM (flexible as needed)

As a Service & Support Administrator (SSA), you help individuals with developmental disabilities live fuller, more independent lives in their communities. You become the person who helps connect the dots between goals, services, providers, and opportunities.
This role is about coordination, advocacy, and impact. You’ll work directly with individuals, families, providers, and community partners to ensure the right supports are in place and working well.
As an SSA, you will coordinate services and supports that help individuals live the lives they choose. Key responsibilities include:
Developing and maintaining Individual Service Plans (ISPs) using person-centered planning
Coordinating assessments and services that support each individual’s goals
Monitoring services to ensure quality, safety, and effectiveness
Working closely with individuals, families, providers, and community partners
Responding to health and welfare concerns when they arise
Maintaining accurate documentation and regulatory compliance
Supporting individuals through ongoing service coordination and advocacy
Participating in an on-call rotation (roughly 2-3 times a year) to address urgent needs
Every day looks a little different. One moment you might be helping someone plan their future goals, the next you’re solving a problem that helps them stay safely supported at home.
Minimum Qualifications
Bachelor’s degree (human services, social work, special education, or related field preferred)
Ability to obtain required Service & Support Administrator certification (at hire)
Valid Ohio driver’s license and reliable transportation
Ability to pass required background checks, drug screen and physical
