Summary of Position:
Comprehensive Behavioral Healthcare, a Porch Light Health company, is seeking a Case Manager for our Lakewood Colorado Location. Porch Light Health is a local leader in outpatient treatment of Opioid Use Disorders and other addictive disorders. We foster a family environment where patients are treated with compassion and honesty. We believe that addiction is a chronic relapsing brain disease. Our staff is passionate about helping other people. We offer Methadone, Medication Assisted Treatment and behavioral health services. We believe there is life beyond addiction and strive to help our patients discover their purpose and community in a healthy environment. We are looking for people who are willing to work in a fast paced medical environment.
The Case Manager will be responsible for patient case management in a substance abuse treatment setting assisting with social, educational and occupational goals. including referrals, care coordination, and community outreach. Case managers link patients with community support, vocational development, housing, mental health and medical care. Case managers must adhere to all state of Colorado and federal regulations and CARF clinical standards,
Key Responsibilities:
-Conduct regular case management sessions.
- Collect and assess social, educational, and vocational information.
- Orient patients to treatment program operations and procedures.
- Assists patients in scheduling appointments and completing paperwork
- Linking patients via referral to community support, vocational development, housing, mental health, and medical care; providing care coordination and follow-up.
- Maintain a working knowledge of substance use disorder theories and treatment.
- Possesses comprehensive knowledge of third-party services for referral in the area.
- Maintains daily, accurate, complete documentation
-Assists in implementing new programs and advising patient needs
- Provide interventions, as needed
- Participates in community and public relations activities, partnering with treatment providers, hospital Emergency Departments, harm reduction organizations, support services, law enforcement, and other partners to educate each about the availability of treatment and ensure effective linkage to services from the community.

- Adhere to all federal, state and local regulations and accrediting agency requirements.
- Commit to diversity and inclusivity in the workplace.
- Assist in monthly clinic operations, as needed.
Education, License/ Certifications and Experience
- Bachelor’s degree in behavioral science or human services preferred
- Knowledge of substance use disorders, treatment methods and current trends in opioid maintenance treatment preferred
- Computer Skills: Microsoft Word, Excel and Microsoft Outlook and Electronic Health Records

- Communication: Able to engage with patients, staff, referral sources, and other stakeholders in a positive and clear manner, and articulate sound business practices
- Coordination: Ability to define issues, analyze problems, evaluate options, and set priorities
- Judgment: Able to make sound decisions and implement or ask for help when needed.
Job Type: Full-Time M-F 6:00am - 2:00pm, in clinic

Compensation: $20 - $26 per hour dependent upon education, experience and location.
