Job Description

SCHEULED HOURS:

MONDAY-FRIDAY 2:30PM-7PM

Saturday and Sunday 7AM-PM

Essential Duties and Responsibilities:

Basic laundry

Shopping and running errands

Prescription medical management

Help with personal hygiene and care

Assisting with meals and nutrition

Help with mobility and transfer

Home maintenance and basic housekeeping

Keeping them company

Reporting and monitoring the patients’ health changes

Other Responsibilities may include:

Communication: Maintaining clear communication with the individual, their family and other health care professionals.

Documentation: Keeping accurate records of care provided and any changes in the individual’s condition.

Safety: Ensuring a safe and comfortable living environment.
