Job Description

Segue Health-- Faith Based Medical Practice

Segue Health, a faith based medical practice, is seeking a compassionate Care Coordinator to join our team serving patients in the Plano, TX area coordinating post discharge transitional care management. This position requires a mix of clinical knowledge, excellent organizational skills, and excellent communication skills.

Qualifications

Must have experience in the medical field

Must have a knowledge of Microsoft Excel and the ability to learn EMR/EHR software

Should have transportation to and from local hospital

Must have a heart to serve others

Daily tasks include:

Working with referring facility to generate list of patients eligible for TCM services

Visiting referrals while in the facility to explain the TCM program and determine if there are any anticipated needs post discharge

Daily logging of referrals and discharges

Creating charts in EMR and uploading face sheet, H&P, and DC Summary

Post discharge calls to patients within 24 hours

Scheduling of patients preferably prior to discharge for in home visits

Our most successful candidates have experience in social work, nursing, and/or home health.

Attention to detail and organizational skills are MOST important.

Must provide proof of vaccinations or meet vaccination requirements per hospital standards

Job Type: Full-time

Salary: $45,000.00 - $55,000.00 per year
