We are looking for a highly organized and proactive Care Coordinator to support the delivery of consistent, high-quality client care for a U.S.-based healthcare services organization. In this role, you will oversee client onboarding, caregiver scheduling, case documentation, and daily operational coordination. You will serve as the primary point of contact for assigned clients, ensuring their services are delivered smoothly and efficiently from onboarding through long-term care engagement.
Duties / Task to perform
• Manage the full client onboarding process, ensuring all documentation and admission agreements are completed before services begin.
• Coordinate caregiver staffing schedules to ensure 100% coverage of authorized service hours.
• Respond quickly to caregiver call-outs, resignations, or client concerns to prevent service interruptions.
• Recruit and onboard new caregivers as needed to maintain full shift coverage.
• Maintain accurate client records, including demographics, preferences, care plans, and incident documentation.
• Process authorization updates and adjust staffing schedules accordingly.
• Manage client discharges while ensuring proper documentation and compliance timelines are met.
• Verify caregiver visits daily and clear prebilling items to ensure accurate payroll and invoicing.
• Act as the primary liaison between clients and the care team, addressing questions and resolving concerns.
• Document all client communications, updates, and incidents in internal systems in real time.
• Monitor operational items such as pending contracts, time-off requests, open timesheets, and fill-in shifts to maintain smooth operations.
• Build strong relationships with caregivers, vendors, and partners to support service quality and referrals.
Requirements:
• Strong organizational and time-management skills with the ability to manage multiple priorities.
• Excellent verbal and written communication skills.
• Strong problem-solving and conflict-resolution abilities.
• Ability to work in a fast-paced, service-oriented environment.
• Proficiency with Microsoft Office and scheduling or workforce management platforms.
• Adaptability in a dynamic and entrepreneurial work environment.
• Previous experience in client services, healthcare coordination, or case management preferred.
• Familiarity with healthcare authorizations and caregiver staffing models is a plus.

Monday - Friday 8:00AM - 4:00 PM (EST)
