Job Description

WE ARE TITANIUM HEALTHCARE

Titanium is a healthcare company that puts heart and compassion above all else. Millions of Americans just aren’t getting the medical care they need. We’re on a mission to change that. For patients that means exceptional support and better care. For providers it means better support and time to focus on patients, and for partners that means higher quality and lower cost.

Join us in our mission!

We are committed to advancing health equity through California’s CalAIM initiative, including the Enhanced Care Management (ECM), Community Supports (CS), and Health Home Program (HHP) programs. We support vulnerable members of our communities by providing high-quality, person-centered care coordination. Our teams are innovative, collaborative, and passionate about improving healthcare delivery.

POSITION SUMMARY

We are seeking a dedicated and adaptable Trainer to deliver engaging and effective training for Titanium Healthcare staff across the ECM, CS, and HHP Programs. The Trainer plays a vital role in preparing staff to provide high-quality, person-centered care while meeting compliance, documentation, and program standards.

The Trainer is responsible for facilitating onboarding, reinforcing best practices, and providing staff with the tools and confidence needed to succeed in their roles. This position emphasizes training delivery and learner support, working closely with the Training Leader and Director of Education & QA to align training content with organizational goals, program requirements, and audit readiness.

At Titanium Healthcare, you will be part of a mission-driven team dedicated to advancing health equity across California.
