FAMILY COORDINATOR
Job Category: Behavioral Interventionist Program
Job Status: Full-time, exempt position
Reports to: Behavioral Interventionist Program Manager
Responsibilities:
Leadership & Case Management
Responsible for family satisfaction within the BI program. Ensure that families served are given proper support, advocacy, resources, and communication in a timely manner and implementing trauma informed care practices.
Complete required assessments with families in a timely manner, scoring correctly, tracking client progress, and utilizing assessments to adapt services to the needs of the client and family.
Attend court appointments and other professional family-based meetings as required.
Work with team in matching BI’s to homes, resolving issues in a timely manner, and participating and engaging in team meetings.
Collaborate and work with BI Coordinator in setting up Meet & Greets, Shadow/Supportive Shifts, Observation Shifts, and other needs.
Assist in planning and facilitating BI Monthly Staff Meetings.
Inform BI Coordinator of family updates, needs, and changes that impact BI services in a timely manner.
Review Incident Reports with BI Coordinator and follow-up with family, if needed.
Complete & facilitate Triage, Admission Meeting, and Meet & Greets for each family within the required time frames.
Complete and submit approved Recommendation and Approval Letters for services in a timely and professional manner.
Create Behavioral Plan for each client, obtaining approval and signatures in a timely manner.
Update and inform families and professional team members when placed on waitlist and during the process of Admission Meeting, approval, and BI hiring process.
Complete all quarterly meetings and biannual assessments with each family and if applicable, BI, within the required time frame.
Write and submit renewals in a timely manner.
Review and update Behavioral Plan with family, professional team as necessary, and BI/BI Coordinator obtaining approval signatures of any changes.
Update SAM client profiles of any and all changes in the required time frame.
Complete End of Month Reports and turn into supervisor by the 1st of the following month.
Complete Case Closure and Aftercare Follow-up meetings, assessments, and documentation in the required time frame.
Outcome Planning/PQI
Complete agency reports and statistics as needed or assigned.
Ensure procedures are in place to track and report program outcomes and quality assurance.
Ensure all resource provider files are in compliance with requirements.
Ensure all data necessary for the KU Research Study is entered into SAM, the electronic health record, and completed in a timely manner.
Partnership with staffing coordinators to complete all Incident Report Reviews within 2 business days of an incident report being completed.
Work collaboratively with branch office to ensure program fidelity.
Ensure that SAM updates and all necessary documentation is uploaded to each client’s SAM profile in a timely manner.
Qualifications & Preferred Skills
Master’s Degree (or Bachelor’s Degree plus two (2) years of experience working in non-profit social service agencies) in social work, psychology, public administration, or a related human services field.
Previous experience working with at-risk populations required.
Familiarity with the child welfare system (preferred).
Demonstrated resourcefulness in setting priorities, proposing new ways of creating efficiencies, and guiding investment in people and systems.
Core understanding of program development, implementation and evaluation.
Outstanding organizational and planning skills.
Solid commitment to the principles of support and advocacy for foster and adoptive families.
Strong verbal communication skills and demonstrated talent to write clearly and persuasively.
Demonstrated leadership, team management, and interpersonal skills. Must have ability to quickly establish rapport and build trust.
The aptitude to collaborate and advocate for positive outcomes, using excellent interpersonal skills in multidisciplinary, diverse, and dynamic professional teams.
Flexible and a self-starter; multi-task while also being highly detail-oriented while meeting all deadlines.
Work as a necessary part of a team effort with colleagues of all demographics.
Model ethical and professional behavior at all times.
Participate in ongoing meetings/professional memberships and trainings as required by the Program Director/President.
Experience working with marginalized communities with a nuanced understanding of systemic oppressions.
