Ben jij een social worker of verpleegkundige die stevig staat in de dynamiek van de stad? Heb jij hart voor mensen met complexe psychiatrische én forensische problematiek en krijg je energie van outreachend werken in de wijk?

In het forensisch FACT-team Utrecht werk je aan herstel én veiligheid. Je beweegt je midden in de samenleving en levert een concrete bijdrage aan stabiliteit, perspectief en risicobeheersing -samen met cliënten, hun netwerk en ketenpartners.

Dit ga jij doen

Als behandelaar werk je outreachend en waar nodig poliklinisch. Je beweegt mee met de cliënt en werkt daar waar het nodig is: thuis, in de wijk, bij instanties of op de poli.

Jij:

• Coördineert de zorg rondom jouw cliënten en bewaakt de voortgang.

• Werkt systemisch en multidisciplinair samen met psychiaters, psychologen, verpleegkundigen, vaktherapeuten en maatschappelijk werkers.

• Zet interventies in zoals diagnostiek, motiverende gespreksvoering, crisisinterventies, herstelondersteuning en risicomanagement.

• Werkt intensief samen met ketenpartners zoals reclassering, gemeenten, woningcorporaties en verslavingszorg.

• Houdt continu de balans tussen zorg, veiligheid en regie binnen een forensische context.

Het Ambulant Centrum Utrecht biedt forensische en intensieve, outreachende psychiatrische zorg aan volwassenen. Jij komt terecht in het forensisch FACT-team: een multidisciplinair, betrokken en professioneel team.

Onze cliënten hebben vaak ernstige psychiatrische aandoeningen in combinatie met verslaving, persoonlijkheidsproblematiek en maatschappelijke uitval. Er kan sprake zijn van (dreigend) normoverschrijdend gedrag en zorg vindt plaats in zowel vrijwillig als justitieel kader.

Dat vraagt om forensische scherpte: oog voor risico’s en veiligheid, zonder het herstelperspectief uit het oog te verliezen. Bij Fivoor gaan behandeling van risico- en delictgedrag en het versterken van herstel op alle levensgebieden hand in hand.
