The Adult Care Manager is responsible for coordinating comprehensive, person-centered care for adults who have complex medical, behavioral health, and psychosocial needs. This role provides community-based care management services, including assessment, care planning, service coordination, and ongoing monitoring to improve health outcomes and quality of life.The Care Manager collaborates with healthcare providers, behavioral health clinicians, social service agencies, and community partners to ensure integrated care across systems. They support individuals with high-risk or complex needs, connect them to resources, and help reduce barriers to care while improving overall health outcomes.
Duties and Responsibilities::
• Conduct comprehensive assessments of medical, behavioral health, and psychosocial needs.
• Identify risks for adverse health outcomes such as hospitalization, emergency visits, or functional decline.
• Develop individualized, person-centered Plans of Care based on client goals, strengths, and barriers.
• Implement and monitor care plans in collaboration with clients and interdisciplinary care teams.
• Review and update care plans regularly based on progress and changing needs.
• Provide health education and self-management support.
• Promote self-advocacy, independence, and wellness.
• Support clients in navigating insurance benefits and eligibility requirements.
• Advocate for timely access to care and removal of service barriers.
• Coordinate care following hospitalizations, emergency visits, or other critical events.
• Facilitate communication among primary care providers, specialists, behavioral health providers, and community supports.
• Maintain accurate, timely, and audit-ready documentation.
• Conduct home visits and community visits as required.
• Collaborate with families, caregivers, and service providers.
Qualifications::
• Bachelor’s degree in Social Work, Psychology, Human Services, Nursing, or related field.
• Minimum 2–3 years of experience working with adults in behavioral health, care coordination, case management, or community health.
• Strong organizational, communication, and problem-solving skills.
• Ability to work independently and within multidisciplinary teams.
• Experience in Medicaid care management programs.
• Knowledge of serious mental illness, substance use disorders, or chronic medical conditions.
• Bilingual in Spanish preferred.
Working Conditions: This position will be located at the Manhattan office at 33 West 60th Street, New York, NY 10023, Suite 600, or any of our other New York State clinics. This is a professional work environment where a computer, desk, chair, and office supplies will be provided onsite at each of our offices.
Physical Requirements: The physical demands described here are representative of those that must be met by an employee in order to successfully perform the essential functions of the role above.
While performing the duties of this job, the employee is regularly required to hear and speak on the phone or computer throughout the day, stand and sit for an extended period of time and be able to write and answer emails in addition to carrying objects no greater than 25 pounds, when needed.
