Administrative Medical Assistant – Care Coordination (Remote)
United States (Remote – preference for Pacific Time availability)

The Opportunity
We’re hiring an Administrative Medical Assistant – Care Coordination to support Akido’s providers, Street Medicine operation teams, and field staff through referrals management, medication coordination, scheduling, and patient record administration.
This is a fully remote, operations-focused MA role ideal for someone who thrives in fast-paced healthcare environments and enjoys organizing care behind the scenes to ensure patients receive timely services, medications, and follow-up.
You will serve as a central connector between patients, providers, pharmacies, specialists, health plans, and Akido’s Street Medicine teams.
What you’ll do
Referrals & Care Coordination
•
Create, submit, and manage specialty referrals and Enhanced Care Management (ECM) referrals in accordance with health plan and internal workflows.
•
Track referral status, authorizations, and outcomes; proactively follow up to resolve delays or missing documentation.
•
Coordinate closely with providers, case managers, and field teams to ensure continuity of care.
Medication & Pharmacy Support
•
Process medication refill requests and route to providers for review and approval.
•
Communicate directly with pharmacies to resolve coverage issues, prior authorizations, medication availability, and patient access barriers.
•
Track medication fulfillment and escalate urgent issues when needed.
Scheduling & Patient Logistics
•
Schedule and coordinate patient appointments across primary care, specialty care, diagnostics, and community partners.
•
Communicate appointment details to patients and field teams (time, location, preparation instructions, transportation needs).
•
Manage reschedules, cancellations, and urgent scheduling requests.
Chart Creation & Documentation
•
Create and maintain patient charts in the EHR and case management platforms.
•
Ensure demographic, insurance, referral, and clinical documentation is accurate and complete.
•
Prepare charts prior to visits and support providers with post-visit documentation workflows.
Team Operations
•
Serve as an administrative point of contact for field teams and clinical staff.
•
Participate in daily huddles and care coordination meetings.
•
Identify process gaps and contribute to improving workflows for referrals, scheduling, and medication management.
Other
•
Duties may evolve with program needs; other job-related tasks as assigned.
Who you are
•
Medical Assistant certificate and current BLS required.
•
1+ year experience as a Medical Assistant, care coordinator, or healthcare administrative role.
•
Strong experience with:
•
EHR systems
•
Referrals management
•
Appointment scheduling
•
Medication refill workflows
•
Highly organized, detail-oriented, and able to manage multiple patient cases simultaneously.
•
Comfortable working across multiple platforms (EHR, referral portals, health plan systems, case management tools).
•
Excellent written and verbal communication skills.
•
Compassionate and professional when working with underserved populations, including individuals experiencing homelessness and/or substance use disorders.
•
Reliable, punctual, collaborative, and self-directed in a remote environment.
•
Bilingual preferred (Spanish a plus).

Work Environment & Physical Demands
This role is fully remote and primarily sedentary. Requires prolonged computer use, frequent phone calls, and digital communication with providers, pharmacies, patients, and care teams.
Hourly pay range
$23—$30 USD
