Inhoud vakgebied:
Als maatschappelijk werker regel en organiseer je zaken op sociaal/ maatschappelijk gebied. Je biedt ondersteuning en begeleiding aan cliënten en/of hun systeem waarbij sprake is van veelal complexe psychosociale en of ernstige psychiatrische problematiek.

Wat kan een maatschappelijk werker betekenen voor de cliënten en de diverse teams:
- Aanvragen/ beheren/ wijzigen/ contacten met en over bewindvoering en mentorschap.

• Multi disciplinaire overleggen (MDO’s) bijwonen en meekijken of daarin op sociaal/ maatschappelijk gebied nog zaken geregeld en/ of opgepakt dienen te worden.

• Bijwonen van behandelevaluaties op consultbasis.

- Regelen van vervoersvoorzieningen via de WMO of Valys.

- Andere contacten met de gemeente inzake wonen/ werken/ inschrijving/ beschermd wonen etc.

• Onderhouden van contacten en overleg met familie en 1e contactpersonen.

• Organiseren van (familie) infoavonden op de diverse afdelingen binnen Vincent van Gogh

• Netwerkcontacten onderhouden met het UWV, zorgverzekeraars, Veilig Thuis, Veiligheidshuis, Woningbouwvereniging, de wijkagent en andere zorgorganisaties.
