About the Role
We are seeking a Specialized Mental Health Caseworker, a role designed to provide dedicated, high-intensity, person-specific support to individuals with complex needs—including developmental disabilities combined with two or more mental health, mood, or personality disorders.
This position is assigned only following clinical recommendation and requires an emotionally resilient, highly skilled, and compassionate professional who can provide stability, safety, and person-directed support.
The role focuses on:
Building strong therapeutic alliances
Promoting emotional regulation, resilience, and well-being
Providing consistent, skilled day-to-day supports
Ensuring health, safety, dignity, and community inclusion
Key Responsibilities
Direct Support & Clinical Implementation
Provide consistent, compassionate support during periods of emotional intensity, dysregulation, or rapid behavioural change.
Follow clinical strategies and treatment recommendations to maintain safety and stability.
Support individuals in daily living, community participation, emotional regulation, and decision-making.
Assist with medication administration, crisis response, and day-to-day life skills.
Person-Directed Planning
Lead the development of Life Plans/Individual Support Plans (ISP), grounded in person-centered and clinically informed practices.
Conduct assessments of living, learning, working, and social skill capacities.
Create and monitor Passport budgets and ensure funds are used appropriately.
Review and update plans every six (6) months or as needed.
Specialized Behavioural & Risk Support
Develop and implement:
Behaviour Support Plans
Individualized Risk Mitigation Plans
Crisis protocols
PRN and safety plans
Conduct data tracking, Symptomatic Charting, and analysis of behavioural trends.
Communicate observations to Program Managers, clinicians, families, and external partners.
Education & Skill Development
Support the delivery of life skills training and community-based learning.
Educate individuals, families, and staff on topics such as:
Nutrition
Medication safety
Independent living skills
Community transportation
Personal care and social development
Collaboration & Advocacy
Work collaboratively with clinicians, physicians, hospitals, mental health services, host families, and support networks.
Advocate for appropriate supports, resources, and system improvements.
Documentation, Records & Compliance
Maintain accurate documentation in ShareVision.
Ensure compliance with QAM, CLGS policies, and safety standards.
Report risks, progress, and variances promptly.

This role requires:
High emotional resilience and rational detachment
Ability to make clinically informed decisions under pressure
Strong problem-solving and risk-assessment skills
Capacity to support individuals during behavioural or emotional escalation
Physical Requirements
Ability to lift 10–50 lbs (depending on location)
Ability to stand, walk, bend, push, and pull
Ability to perform Safe Management physical interventions (annually trained)
Ability to respond quickly in emergencies or crisis situations
Schedule & Conditions
Rotating shifts: days, evenings, weekends (as per Collective Agreement)
Call-ins as required
Travel for appointments and community activities
Use of personal and agency vehicles
Overtime may be required
Qualifications
Experience supporting individuals with developmental disabilities AND complex mental health needs
Knowledge of psychosocial rehabilitation, trauma-informed care, and behaviour support
Ability to implement clinical recommendations
Strong documentation, communication, and teamwork skills
Valid driver’s license and reliable vehicle
Developmental Service Worker Diploma or related field of study
Valid First Aid, CPR certificate, and Safe Management training
Work Hours:
The hours of work are scheduled 80 hours bi-weekly with the potential of more shifts as they become available (Full-time)
